After you pass the course with an 84% or higher, you must fill out the course evaluation form. There is a button in your student workroom that reads "CE Credit Form". Click it and fill out the form. Then, you will be able to print your CE certificate by clicking the "Print CME Certificate" button. This button only appears after you fill out the CE credit form.

Option 1: Mail your provider certificate and a copy of your receipt to us.

• The name and order number as on your sales receipt.
• The name of your employer.
• The name and contact information (email and phone) for your supervisor or HR department that is not accepting our certificate.
